The cover story for the summer 2023 edition looks into the Loy-Lange Box Co. explosion in April 2017, which killed four people, and the ensuing incident report from the US Chemical Safety and Hazard Investigation Board (CSB), which was released in July 2022. The story examines how the CSB conducts an investigation, the status of the recommendations, and how the industry can learn from the tragedy.
